2016-3Q: SIMPLer: EUP Bank Restriction Options

New options have been added to the SIMPLer system to allow Operator configure bank account functionality in the EUP per customer.

The options shown at Fig. 1 can be found under the customer record on Modify Customer -> Banking Details (End User Portal Settings section).
